Care records
One record per person: profile, contacts, risk flags, assessments, a versioned care plan and an append-only timeline that inspection can read without a translator.
What it holds
Profile with funding route, GP, access details (read-audited) and key-holder contacts.
Critical flags that outrank everything else on screen and cannot be collapsed in the carer app.
Assessment forms from a versioned template set; signed assessments are superseded, never edited.
Care plans with needs, intended outcomes and per-visit tasks; a published plan is pinned to each visit so a mid-visit change never rewrites a visit in progress.
How it is kept honest
The timeline is append-only. Corrections are new entries that reference the original.
Every write records who, in which role, at what time, and what changed.
Reads of sensitive panels (access details, medication, incident narratives) are logged too.
